ACA Double-Bonus Statutory Cap Compression & D-SNP AIP Integration Mandates
Under SSA § 1853(n)(4), Medicare Advantage county benchmark rates—including the statutory 5.0% Quality
Bonus Payment (QBP) uplift and 10.0% double bonus in qualifying high-penetration counties—are legally
capped at the pre-ACA statutory ceiling. Empirical warehouse modeling of CMS CY 2027 Rate Books reveals
that 1,269 counties nationwide suffer statutory cap truncation. In severe cases such as
Anoka County, MN (FIPS 27003, 41,298 enrolled lives), the pre-ACA cap of $1,204.07 completely
eliminates the double bonus: the benchmark remains frozen at $1,231.46 whether rated 0★ or 5★,
compressing the effective bonus to 0.0% and truncating $123.15 PMPM in revenue.
In Hennepin County, MN (FIPS 27053, 123,575 lives), cap compression truncates $97.93 PMPM,
leaving an effective bonus of just 2.41%.
By contrast, high-benchmark southern markets retain substantial headroom: in Broward County, FL
[C-Q29-001], the pre-ACA ceiling of $1,720.40 sits comfortably
above the 5.0★ benchmark of $1,406.23, yielding zero compression ($0.00 PMPM). Similarly,
Miami-Dade County [C-Q29-004] maintains a statutory ceiling of $2,405.24
against a $1,478.87 benchmark. This geographic disparity produces an asymmetric margin squeeze: Midwest and
Northeast regional plans face severe revenue compression that constrains supplemental benefit funding.
Concurrently, CMS’s enforcement of Applicable Integrated Plan (AIP) exclusivity under 42 CFR § 422.514 is
reordering the dual-eligible market. While legacy non-AIP D-SNPs such as Humana Gold Plus H1036-304
[C-Q26-001] (38,402 market lives) contracted by -1.18% YoY,
integrated AIP entrants experienced rapid expansion: UnitedHealthcare Preferred Dual Complete H1045-063
[C-Q26-002] (27,213 market lives) grew +5.69% YoY, and newly
integrated HealthSun MediSun Full Dual Extra H5431-026 [C-Q26-003] surged
+100.0% YoY across 21,298 enrolled lives.

The 3.750 CAI Rounding Cliff & 2028–2029 Measure Sunset Headwinds
Under CMS Star Ratings rounding methodology (42 CFR § 422.166(f)(1)), an unrounded composite summary score of
3.749 rounds down to 3.5★, eliminating the 5% QBP benchmark uplift and reducing the rebate pool
from 65% to 50%; a score of 3.750 rounds up to 4.0★. In this razor-thin margin, the Categorical
Adjustment Index (CAI)—an additive adjustment ranging from -0.036 to +0.115 based on low-income
and disability quintiles (FAC 1–8)—acts as the ultimate arbiter of contract solvency.
The empirical evidence from Star Year 2027 is unforgiving: Humana contract H5619
[C-Q24-001] posted an unadjusted summary score of 3.718. Despite receiving a
favorable CAI adjustment of +0.031, its final score reached 3.749 (3.5★)—missing
4.0★ QBP qualification by exactly 0.001 points (-0.001 cliff distance). Even more severely,
negative CAI adjustments demoted otherwise qualifying plans: Humana H5377
[C-Q24-002] achieved an unadjusted 3.750, but a -0.002 CAI penalty
dragged its final score to 3.748 (3.5★). Similarly, Essence Healthcare H6200
[C-Q24-003] fell from an unadjusted 3.763 to 3.748 (3.5★) due to
a -0.015 CAI drag. Conversely, Devoted Health H7355
[C-Q24-004] leveraged a +0.031 CAI lift to turn an unadjusted
3.724 into 3.755 (4.0★), preserving $10M+ in QBP funding.
Compounding this fragility, pro-forma simulation of the CY 2027 Final Rule (91 FR 17384) measure sunset schedule
demonstrates acute future headwinds. Across scheduled measure eliminations (3 measures in 2028 and 10 additional
measures in 2029), contracts heavily reliant on legacy process measures face severe rating drops. Alignment Health
Plan H7074 [C-Q25-001] collapses from a baseline of
4.0★ (3.929 weighted avg) to a pro-forma 2028 score of 3.5★ (3.500, -0.5 pts)
and a 2029 score of 3.5★ (3.333, -0.5 pts), triggering a statutory
CLIFF_BREACH_LOSS. Blue Cross Blue Shield of Alabama H0104
[C-Q25-007] maintains 4.0★ in 2028 but breaches the cliff in 2029 to 3.5★.

Voluntary Disenrollment Root Causes & The 42 CFR § 422.166 Star Penalty Loop
Analysis of 12,824 contract-level disenrollment records across Star Years 2023–2026 uncovers a direct causal
nexus between network benefit retrenchment and catastrophic beneficiary churn. Across the 4 statutory causes
monitored by CMS, "Problems with Coverage of Doctors and Hospitals" and
"Financial Reasons" represent the overwhelming drivers of voluntary flight.
For flagship regional contracts, churn metrics have breached critical thresholds: Humana H1036
[C-Q23-001] (206,316 market lives, 4.5★) registered a voluntary disenrollment rate
of 46.0%, driven by doctor/hospital network friction (22.0%), financial causes
(13.0%), and administrative navigation barriers (11.0%), placing it in the
CRITICAL financial churn risk tier. UnitedHealthcare H1045
[C-Q23-002] reached 44.0% total disenrollment (17.0% doctor/hospital,
10.0% financial). For sub-4.0★ plans, the churn spiral accelerates: Humana H5216
[C-Q23-006] (3.5★) suffered 62.0% total disenrollment (24.0% doctor/hospital,
18.0% financial), while Aetna H1609 [C-Q23-008] reached
66.0% total voluntary turnover (37.0% doctor/hospital coverage friction).
Under 42 CFR § 422.166, this churn feeds directly into Measure C27 (Members Choosing to Leave the Plan)
and CAHPS satisfaction measures. Health plans attempting to defend statutory margins through aggressive benefit
cuts trigger an immediate spike in disenrollment, which degrades C27 and CAHPS ratings, resulting in sub-4.0★ demotion
and the permanent loss of the 65% rebate pool funding required to stabilize bids.

CY 2026 → CY 2027 PBP Benefit Pullback & The Margin Defense Posture
The public release of CMS CY 2027 Plan Benefit Package (PBP) files (14,487 plans) confirms widespread
supplemental benefit pullback as health plans react to CMS-HCC V28 risk adjustment phase-in and Star cut-point
tightening. In competitive benchmark markets such as South Florida (Q22), 50% of evaluated plans
(25 of 50) executed an AGGRESSIVE_PULLBACK strategy, while another 18% (9 of 50)
implemented a MODERATE_PULLBACK, resulting in 68% of competing offerings reducing
benefit richness.
Humana’s market-leading HMO plan, H1036-065C [C-Q22-001]
(48,629 enrolled lives), saw its Benefit Richness Score plummet from 80.6 to 65.7 (-14.9 pts).
The plan completely eliminated its annual flex card allowance (-$1,435.00 delta) and reduced
hearing aid allowances by -$750.00. Similarly, Humana H1036-062C
[C-Q22-007] dropped from 80.6 to 62.0 (-18.6 pts), slashing
flex card dollars by -$1,425.00, hearing allowances by -$1,250.00, and increasing
emergency copays. Humana Dual Select D-SNP H1036-304 [C-Q22-002]
cut richness by -11.3 pts (86.3 to 75.0), reducing hearing allowances by -$1,500.00.
In contrast, UnitedHealthcare’s MedicareMax H5420-001 [C-Q22-003]
executed a counter-cyclical BENEFIT_EXPANSION_ENHANCEMENT strategy, increasing its richness score from
37.9 to 38.2 (+0.3 pts) while expanding dental and hearing allowances by +$700.00,
positioning itself to capture members displaced by Humana’s supplemental concessions.

The CY 2027 Tukey 3.0× IQR Cut-Point Shock & The $104.22M H3359 Cliff
Under 42 CFR § 422.166(a)(2)(i), CMS’s second-year enforcement of Tukey 3.0× IQR outer-fence
outlier deletion truncated left-tail scores across 45 of 86 Star measures (+2.87 percentage
points mean upward threshold shift before the 5.0% guardrail cap). Across the 515 rated contracts
in both Star Years 2026 and 2027, 4.0+ Star QBP qualification contracted from 48.0% to 43.3% (-4.7 pts).
For flagship regional HMO Healthfirst (H3359), a 0.5-star compression from 4.0★ to 3.5★
across 214,193 enrolled members places $104,224,251 in Gross 5% County Benchmark Uplift
and $67,745,763 in Net 65% Rebate Benefit Pool dollars at immediate risk entering CY 2028 bid submission.

100% CMS-HCC V28 Phase-In: Quantifying the -$753.03 PMPY Actuarial Drag
Payment Year 2026 completes CMS’s statutory transition from blended 67% V28 / 33% V24 risk adjustment
to 100% CMS-HCC V28. The removal of 2,294 diagnosis codes and re-constraining of diabetes,
vascular, and behavioral HCC coefficients imposes an empirical -4.20% risk score drag on
high-D-SNP urban portfolios—translating to -$161,293,519 in annual risk-adjusted revenue
headwind for H3359 (-$753.03 PMPY).

SciPy MILP Knapsack Allocation: Recovering 4.0★ Under Quality Improvement Hold Harmless
Rather than dispersing clinical outreach budgets across low-leverage chart chases, Maven’s
Mixed-Integer Linear Programming (MILP) optimizer solves the exact minimum-cost measure intervention
vector incorporating contract-specific Categorical Adjustment Index (CAI) and 42 CFR § 422.166(g)(1)
Quality Improvement (C26/D04) hold-harmless mechanics. For H3359, a targeted $2.05M clinical
deployment across 11 measures lifts the unrounded Star score from 3.389 to 3.758 (4.0★)—delivering
a 50.8× gross QBP ROI.
